Pharmacist – Hospital Pharmacy Services

About the role

Lakeridge Health is seeking a dedicated Pharmacist to join its Pharmacy Services team in the Ajax‑Pickering region. You will work within a multidisciplinary environment to ensure safe, effective medication use for patients across all care settings.

Key responsibilities

• Provide direct patient care by resolving and preventing drug‑related problems.
• Participate in medication reconciliation and patient education initiatives.
• Teach pharmacy students and deliver education to nursing and medical staff.
• Support the drug distribution system and maintain compliance with safety programs.

Required profile

• Current registration in good standing with the Ontario College of Pharmacists (Part A).
• Strong clinical assessment and medication counseling abilities.
• Excellent organizational and communication skills.
• Ability to work effectively within an interprofessional team.
• Commitment to Lakeridge Health’s core values and safety programs.
