Pharmacist – Clinical Coverage

About the role

The Pharmacist will join Brant Community Healthcare System to provide clinical pharmacy coverage across multiple inpatient and outpatient areas. Reporting to the Director of Pharmacy Services, the role supports patient care at Brantford General Hospital and the Willett urgent‑care centre in Paris, Ontario.

Key responsibilities

• Deliver medication therapy management and clinical pharmacy services in designated units.
• Ensure accurate dispensing, compounding, and distribution of medications.
• Collaborate with interdisciplinary teams to optimize patient outcomes.
• Maintain compliance with provincial pharmacy regulations and hospital policies.
• Participate in medication safety initiatives and quality improvement projects.

Required profile

• Registered Pharmacist with a valid Ontario license.
• Strong clinical knowledge and ability to work in a fast‑paced hospital environment.
• Excellent communication and teamwork skills.
• Commitment to patient safety and continuous learning.
