About the role
The Primary Care Nurse Practitioner role focuses on comprehensive primary care, health promotion, and disease prevention across the lifespan. You will work within the Primary Care Connection Clinic (PCCC) to attach individuals to primary care services while contributing to education and consultation activities.
Key responsibilities
- Perform assessments, diagnoses, treatments, health education, and counseling in line with College of Nurses of Ontario standards.
- Apply a Social Determinants of Health perspective to improve outcomes and reduce health inequities.
- Utilize a Harm Reduction approach with clients exhibiting risky behaviors.
- Triage client needs via phone or in‑person consultations.
- Maintain accurate clinical notes and adhere to professional and organizational policies.
- Develop, implement, monitor, and evaluate health promotion and disease prevention programs.
- Work regular and extended hours, including at least one evening per week.
- Participate in quality‑improvement initiatives, data collection, analysis, and research as required.
Required profile
- Completion of a recognized Primary Care Nurse Practitioner program and a Master’s degree in nursing (or equivalent).
- Current College of Nurses of Ontario Extended Class certification in good standing.
- Minimum two years of Primary Care NP experience working with all ages in a community health setting.
- Valid Ontario driver’s licence and ability to obtain a Police Vulnerable Sector Check.
- Knowledge of Ontario health delivery systems, community and social services, and relevant legislation.
- Demonstrated collaborative teamwork and leadership in professional practice.
