Nurse Practitioner (NP) – Long Term Care

About the role

RhynoCare is seeking an experienced Nurse Practitioner to provide high‑quality, compassionate care to residents in long‑term care facilities in Markham, Ontario. The role combines direct patient care with research and education responsibilities.

Key responsibilities

• Deliver primary health care, manage acute and chronic conditions, and provide health promotion and disease prevention for LTC residents.
• Perform comprehensive health assessments, order and interpret diagnostic investigations, and develop person‑centered care plans.
• Provide on‑call coverage, lead clinical teams, and support discharge planning, palliative care, and dementia care.
• Dedicate 80% of time to direct resident care and 20% to research, education, and quality improvement initiatives.

Required profile

• Registered with the College of Nurses of Ontario (CNO) in the Extended Class.
• Bachelor’s degree in Nursing (BScN); Master’s degree (MSN) with NP specialization preferred.
• Membership with the Nurse Practitioners' Association of Ontario (preferred) and professional liability insurance.
• Current CPR certification, Vulnerable Sector Check, and required immunizations.

Required skills

• Clinical assessment and diagnostic reasoning within full CNO scope of practice.
• Ability to develop and implement person‑centered care plans and manage complex medical issues.
