Nurse Practitioner – Long Term Care (St. Catharines)

About the role

RhynoCare is seeking an experienced Nurse Practitioner (NP) to provide primary health care in Long Term Care facilities in St. Catharines, ON. The position is hired as an Independent Contractor, offering competitive compensation, flexible scheduling, and ongoing support to focus on delivering exceptional resident care.

Key responsibilities

• Provide comprehensive primary health care services, including assessment, diagnosis, treatment, and follow‑up for acute and chronic conditions within full NP scope.
• Conduct comprehensive health assessments covering physical, psychosocial, emotional, cultural, and functional dimensions.
• Order, interpret, and evaluate diagnostic investigations using evidence‑based clinical reasoning.
• Develop and implement person‑centered care plans in collaboration with residents, families, and Substitute Decision Makers.
• Collaborate with physicians, Directors of Care, and interdisciplinary teams on admissions, discharges, palliative care, pain management, dementia care, and behavior management.
• Participate in resident care conferences, interdisciplinary rounds, quality improvement initiatives, and education/mentorship activities.

Required profile

• Current registration with the College of Nurses of Ontario (CNO) in the Extended Class (Primary Health Care or Adult) and entitlement to practice.
• Bachelor of Science in Nursing (BScN) required; Master of Science in Nursing (MSN) with NP specialization preferred.
• At least one year of clinical experience in geriatric, rehabilitation, or Long‑Term Care nursing preferred.
• Availability for on‑site work with flexibility for evenings, weekends, and on‑call coverage.
• Working knowledge of Long‑Term Care legislation, regulations, and standards of practice.
