Nurse Practitioner – Long-Term Care Leadership

About the role

Lead clinical excellence and mentor healthcare teams in a long‑term care facility, delivering resident‑centred care and supporting families daily.

Key responsibilities

• Provide clinical leadership, mentorship and guidance to RNs, RPNs, PSWs and other care team members.
• Conduct comprehensive assessments, develop individualized care plans and ensure high‑quality resident outcomes.
• Collaborate with attending physicians and interdisciplinary teams to coordinate treatment and address clinical concerns.
• Maintain accurate clinical records, support admission processes and ensure regulatory compliance, including the Fixing Long‑Term Care Act, 2021.
• Identify quality‑improvement opportunities and promote a culture of safety and continuous improvement.

Required profile

• Nurse Practitioner designation and registration in good standing with the College of Nurses of Ontario.
• Experience as a Nurse Practitioner, preferably in long‑term care or with older adults.
• Proven ability to provide clinical leadership, mentorship or supervision.
• Strong critical‑thinking, assessment and clinical decision‑making skills.
• Excellent communication and interpersonal abilities to build positive relationships with residents, families and the care team.
