Hospital Clinical Pharmacist

About the role
The Hospital Clinical Pharmacist provides direct patient‑care services at Qikiqtani General Hospital and Kivalliq Health Centre. Reporting to the Territorial Director of Pharmacy, the pharmacist acts as a drug‑information resource for all health centres and long‑term‑care facilities across Nunavut.
Key responsibilities
- Review medication orders for appropriateness, safety and therapeutic effectiveness.
- Identify, resolve and prevent drug‑therapy problems through patient‑focused interventions.
- Perform medication reconciliation, pharmacokinetic/therapeutic drug monitoring and discharge planning.
- Serve as liaison between patient‑care areas and the pharmacy to resolve medication‑related issues.
- Document clinical activities, interventions and recommendations in the client health record.
- Respond to complex drug‑information requests using primary literature and comprehensive databases.
- Audit pharmacy services and programs as required.
Required profile
- Resident of Iqaluit eligible to work in Nunavut.
- Ability to work full‑time in a hospital/health‑centre environment.
- Strong collaboration skills with prescribers, nursing, laboratory and engineering/biomedical teams.
- Commitment to improving patient safety and medication outcomes.
Required skills
- Medication order review.

- Drug‑information research and database use.
- Clinical documentation.

- Permanent full‑time position.
- Recruitment bonuses: $3,000 at start, $3,000 at 6 months, $5,000 at 1 year, $7,000 at 2 years.
