Health Manager

About the role

The Health Manager leads the clinical and community health portfolio for the Yuułuʔiłʔatḥ Government Health & Social Services department. Reporting to the Director, the role ensures safe, culturally grounded delivery of home and community care, primary health support, chronic disease management and health promotion across Hitacu, Ucluelet and Port Alberni.

Key responsibilities

• Plan, deliver and evaluate community and home care, primary care support, chronic disease and palliative care programs, ensuring they are culturally safe and trauma‑informed.
• Provide clinical governance, overseeing nursing practice, medication management, infection prevention and documentation in line with BCCNM, FNHA and Island Health standards.
• Supervise and develop the nursing team, health care assistants, homemakers and patient transport staff, including recruitment, performance reviews and wellness support.
• Manage patient travel services, maintaining trip logs and reimbursements in accordance with FNHA Medical Transportation guidelines.
• Maintain partnerships with FNHA, Island Health, physicians and community organisations, representing YG Health at inter‑agency meetings and supporting citizens in navigating health systems.
• Oversee the health portfolio budget, funding agreements and reporting, ensuring value for money and compliance with financial policies.

Required profile

• Bachelor’s degree in Nursing, Health Administration, Public Health or related field (or equivalent education and management experience).
• Minimum five years of progressive experience in community health or home care, including at least two years supervising clinical staff.
• Working knowledge of the BC health system, FNHA, Island Health and medical transportation processes.
• Valid BC Class 5 driver’s licence and reliable transportation.
• Successful Criminal Record Check with vulnerable sector screening.
