Acute Care Nurse Practitioner – Community Health Care

About the role

We are seeking an experienced Acute Care Nurse Practitioner to join our community health care team, delivering fast‑paced inpatient and outpatient services across emergency, intensive and urgent care settings. The role focuses on providing active treatment, coordinating care and ensuring patients are discharged safely once stable.

Key responsibilities

• Deliver acute care services in emergency rooms, intensive care units and urgent care clinics.
• Assess, diagnose and manage patient conditions, interpreting diagnostic tests and results.
• Coordinate multidisciplinary care plans to achieve timely patient discharge.
• Educate patients and families on treatment plans and post‑discharge care.
• Maintain high standards of patient safety and quality throughout the care continuum.

Required profile

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) degree.
• Current, unrestricted Nurse Practitioner license in the state of practice.
• Certification as an Acute Care Nurse Practitioner (ACNP) or equivalent.
• Minimum three years of acute‑care Nurse Practitioner experience.
• BLS and ACLS certifications.
• Strong diagnostic and decision‑making abilities.
• Excellent communication and patient‑education skills.
