Transition Coordinator – Pediatrics RN IV

About the role

The Transition Coordinator – Pediatrics RN IV reports to the Manager of Infection Prevention and Control & Clinical Collaborative Practice. This role supports collaboration, patient flow and family and staff support between Health and Transition Services, Shared Health Children’s Hospital and community‑based care. It involves on‑site responsibilities at both locations, focusing on early identification, care planning and resource navigation for children with developmental disabilities.

Key responsibilities

• Build relationships and provide education to care teams and leadership at the Children’s Hospital on St.Amant’s mission, vision and values.
• Participate with multiple care teams to support early identification and care planning for eligible children.
• Deliver initial and ongoing family education and support at both hospital and community sites.
• Act as a resource for the defined pediatric patient population, navigating health and social systems for timely access and discharge planning.
• Identify staff education needs, develop communication pathways and maintain efficient patient flow between sites.

Required profile

• Graduate of an approved Registered Nursing program with a Baccalaureate degree in Nursing and RN IV classification.
• Minimum two years recent clinical experience, including interdisciplinary care planning, complex discharge planning and pediatric care.
• Experience working with individuals with intellectual and developmental disabilities and medical complexity.
• Valid Class 5 Manitoba driver’s licence and access to a personal vehicle.
• Knowledge of nursing standards, ethics, relevant legislation and hospital policies.

Required skills

• Proficiency with word processing applications.
• Proficiency with spreadsheet software.
• Experience using databases.
• Experience with electronic medical records (EMR) systems.
