Service Hub Navigator

About the role

The Service Hub Navigator conducts evidence‑based screening and assessments for individuals accessing the Kenora Emergency Shelter, making recommendations for services and appropriate housing levels. This role works on‑site within the shelter, collaborating with community partners and reporting to the Clinical Service Hub Team Lead and CEO.

Key responsibilities

• Engage assertively with shelter clients to connect them to services and conduct community outreach.
• Assess housing needs using VI‑SPDAT, screen patrons with OCAN, and make service recommendations.
• Develop individualized recovery plans, continuously assess risk, and provide advocacy, counseling, and side‑by‑side support.
• Maintain client files, collect statistical data, and participate in staff meetings, committees, and professional development.

Required profile

• BSW preferred or equivalent experience.
• Knowledge of mental health issues, dual diagnosis, poverty, homelessness, and trauma‑informed approaches.
• Strong communication, assessment, case management, and advocacy skills.
• Ability to work independently, professionally, and with cultural competence.
