Crisis Stabilization Worker

About the role

The Crisis Stabilization Worker provides psychosocial support to adults experiencing homelessness, mental health, and addiction crises at the HART Hub in Kenora. Working closely with the integrated care team, the role focuses on short‑term stabilization and linking clients to longer‑term services.

Key responsibilities

• Conduct mental health and addiction assessments with nursing staff to identify risk factors and de‑escalation opportunities.
• Establish therapeutic relationships and provide short‑term follow‑up to bridge gaps between referral and longer‑term supports such as Safe Bed programs or hospitals.
• Advocate for client access to services, engage emergency services when needed, and coordinate with community resources.
• Deliver brief psychotherapeutic interventions, health teaching, and coping‑skill development.
• Screen clients using approved tools (e.g., VI‑SPDAT, OCAN) and triage them to appropriate on‑site or external services.

Required profile

• BSW, MSW or registration with the College of Nurses of Ontario; eligible for regulatory college registration.
• Experience working with homeless, marginalized adults with mental health and addiction challenges.
• Training in crisis intervention and resolution, with strong knowledge of community resources.
• Excellent verbal, written, and documentation skills; culturally competent and trauma‑informed.
• Valid driver’s licence and ability to work independently and as part of a team.
