Pharmacist – Primary Care Team

About the role

The Queen's Family Health Team (QFHT) in Kingston is seeking a licensed pharmacist to join its multidisciplinary primary‑care clinic. Reporting to the Clinic Manager and Lead Physician, the pharmacist will deliver patient‑centred pharmaceutical care, optimise medication therapy and support the interdisciplinary team.

Key responsibilities

• Develop therapeutic and management plans for patients with chronic diseases such as diabetes, hypertension, dyslipidaemia, heart failure and atrial fibrillation.
• Participate in planning, implementation and evaluation of chronic‑disease, health‑promotion and illness‑prevention programs, including anticoagulation management and medication reconciliation.
• Communicate with hospital and community pharmacists and other team members to ensure seamless medication transitions across care sites.
• Conduct medication assessments and counselling for older adults with chronic conditions, patients on multiple drugs, and recently discharged patients.
• Provide up‑to‑date drug information, guidelines and evidence‑based recommendations to physicians, residents and clinic staff.
• Contribute to the education of family‑medicine residents and other learners on medication safety and reconciliation.
• Deliver individual or group patient education sessions on medication use and health‑issue management.

Required profile

• Licensed pharmacist in good standing.
• Ability to work collaboratively within an interdisciplinary primary‑care team.
• Strong communication skills for interacting with patients, physicians, residents and other health‑care professionals.
• Commitment to continuous education and evidence‑based practice.
