Pharmacist – Community Hospital

About the role

The Pharmacist will join Brant Community Healthcare System, a two‑site community hospital network serving Brantford and Paris, Ontario. This position supports both acute‑care and urgent‑care pharmacy services, ensuring safe and effective medication management for patients across the system.

Key responsibilities

• Prepare, dispense, and verify medications for inpatient and outpatient settings.
• Collaborate with physicians, nurses, and allied health professionals to optimize pharmacotherapy.
• Provide medication counseling and education to patients and caregivers.
• Participate in medication safety initiatives, adverse drug reaction monitoring, and formulary management.
• Support urgent‑care and transitional‑care services at the Willett site.

Required profile

• Registered Pharmacist with a valid license to practice in Ontario.
• Commitment to patient‑first care and teamwork.
• Strong communication skills and ability to work in an interdisciplinary environment.

Required skills

• Medication dispensing and verification.
• Pharmacy compounding and sterile preparation.
• Knowledge of hospital pharmacy workflows and electronic medication records.
