Pharmacist – Community Hospital

About the role

The Brant Community Healthcare System (BCHS) is seeking a dedicated pharmacist to join its two‑site community hospital network in Brantford and Paris, Ontario. You will play a vital role in ensuring safe, effective medication management for both inpatient and outpatient populations.

Key responsibilities

• Prepare, dispense, and verify medications in accordance with hospital policies and regulatory standards.
• Collaborate with physicians, nurses, and allied health professionals to optimize pharmacotherapy.
• Provide medication counseling and education to patients and caregivers.
• Participate in medication safety initiatives, formulary management, and clinical rounds.
• Maintain accurate pharmacy records and ensure compliance with documentation requirements.

Required profile

• Registered Pharmacist (R.Ph.) with a valid license to practice in Ontario.
• Strong understanding of hospital pharmacy operations and medication safety.
• Excellent communication and teamwork skills.
• Commitment to continuous learning and professional development.
