Pharmacist – Clinical & Patient Care

About the role

Mint Health + Drugs is seeking a qualified Pharmacist to join its growing team. You will play a key part in delivering exceptional patient care, supporting operational excellence, and contributing to a compassionate healthcare environment.

Key responsibilities

• Assess patient health, manage medication therapy for chronic and acute conditions, and provide medication counseling.
• Administer vaccinations, adapt prescriptions, and manage drug therapy to prevent and treat health disorders.
• Prepare, package, and dispense medications with accurate dosage, labeling, and storage.
• Collaborate with multidisciplinary teams in clinics or hospitals to optimize patient care and transition plans.
• Serve as a drug information resource for patients and healthcare professionals.

Required profile

• Bachelor of Science in Pharmacy or PharmD.
• Active or eligible registration with the applicable provincial pharmacy regulatory body.
• Experience in a hospital setting or direct patient care preferred.
• Successful Criminal Record Check and Vulnerable Sector Search.
• Strong clinical assessment and communication abilities; capable of working independently or within a team.
• Additional Prescribing Authorization (APA) preferred.

Required skills

• Electronic health records (e.g., Kroll).
• Medication order entry systems.
• Vaccination administration.
