Patient Navigator – Chronic Care

About the role

Join Sailor Health as a remote Patient Navigator supporting chronic care patients across Canada. You will guide existing patients through advocacy services, ensuring they receive coordinated, compassionate care beyond their therapy sessions.

Key responsibilities

• Conduct introductory calls with patients interested in advocacy services, following a structured intake process.
• Guide patients through the advocacy onboarding journey with warmth, clarity, and attention to detail.
• Coordinate closely with the advocacy team to ensure smooth transitions and follow‑through.
• Manage patient communications across multiple stages, including follow‑ups, missed calls, and reassignments.
• Maintain accurate records using Google Workspace, EHR platforms (e.g., Healthie), and CRM tools like HubSpot.

Required profile

• Empathetic and clear communicator who can make patients feel at ease.
• Highly organized and detail‑oriented, able to manage multiple patients simultaneously.
• Comfortable with technology and quick to learn new tools.
• Mission‑aligned with a passion for improving care for older adults.

Required skills

• Proficiency with Google Workspace.
• Experience using EHR platforms such as Healthie.
• Familiarity with CRM systems, specifically HubSpot.
