Nurse Practitioner – Primary Care (Full‑Time)

About the role

The Shkagamik‑Kwe Health Centre is seeking a full‑time Nurse Practitioner to provide comprehensive primary health care to individuals of all ages. The role focuses on health promotion, disease prevention, and management of chronic and acute conditions within a culturally respectful, Indigenous‑focused environment.

Key responsibilities

• Conduct health assessments, diagnose, treat and follow‑up with clients across the lifespan.
• Manage chronic diseases such as diabetes, hypertension and respiratory conditions in partnership with patients and caregivers.
• Perform procedures and diagnostic tests within scope of practice and prescribe medications according to guidelines.
• Support mental health through counseling, assessments and coordination with mental health professionals.
• Integrate traditional healing practices, collaborating with Elders, Traditional Healers and Knowledge Keepers.
• Lead community health initiatives, deliver education and participate in workshops and events.
• Work collaboratively with a multidisciplinary team including physicians, traditional healers and other health professionals.
• Maintain accurate client records using electronic medical record systems.

Required profile

• Master’s degree in Nursing or equivalent Nurse Practitioner program.
• Current registration with the College of Nurses of Ontario as a Nurse Practitioner (Primary Health Care).
• Current CPR and First Aid certification.
• Minimum two years of NP experience in primary or community health settings (preferred).
• Experience working with Indigenous communities or in cross‑cultural environments (asset).
• Strong understanding of Indigenous health issues, social determinants of health and trauma‑informed care.

Required skills
