Pharmacist – Community Hospital

About the role

The Pharmacist will join Brant Community Healthcare System’s acute‑care and urgent‑care sites, providing safe, effective medication management for patients across Brantford and Paris, Ontario. This role supports the hospital’s mission to lead beyond care by delivering high‑quality pharmaceutical services in a collaborative, patient‑first environment.

Key responsibilities

• Dispense prescription medications and ensure accuracy, safety, and regulatory compliance.
• Provide medication counseling and education to patients, families, and healthcare staff.
• Collaborate with physicians, nurses, and allied health professionals to optimise drug therapy.
• Maintain inventory, manage drug procurement, and oversee controlled substance handling.
• Participate in clinical rounds, medication reconciliation, and quality‑improvement initiatives.

Required profile

• Pharmacy degree from an accredited program and active registration with the Ontario College of Pharmacists.
• Experience in an acute‑care or hospital pharmacy setting is preferred.
• Strong knowledge of drug interactions, dosing guidelines, and pharmacy law.
• Excellent communication skills and ability to work within interdisciplinary teams.
