Community Registered Practical Nurse (RPN) – 1‑Year Maternity Leave

About the role

This is a new, 1‑year maternity‑leave contract for a General Visiting Registered Practical Nurse (RPN) providing scheduled home visits to patients of all ages across a community setting. You will help patients remain comfortable at home by delivering personalized care during weekday shifts (8 am‑5 pm) and every third weekend.

Key responsibilities

• Use appropriate assessment tools and techniques to evaluate each patient’s individualized needs.
• Develop, execute, and continuously monitor personalized care plans.
• Engage patients collaboratively, building rapport and trust.
• Maintain and update professional patient records for every encounter.
• Provide on‑call coverage, including after‑hours, to ensure patient safety.

Required profile

• Current registration with the CNO as a Registered Practical Nurse in good standing.
• Valid driver’s licence and access to a reliable vehicle.
• Reliability, flexibility, and willingness to travel within the designated area.
• Passion for making a difference in the community.
• Experience delivering community care is an asset.
