Registered Nurse – Community Visiting (Full‑time)

About the role

This full‑time, 1‑year fixed‑term position is for a General Visiting Registered Nurse who will provide in‑home care to patients of all ages across a designated area. You will work weekdays from 8 am to 5 pm with every third weekend off, supporting patients to stay comfortable at home.

Key responsibilities

• Use appropriate assessment tools and techniques to evaluate each patient’s individualized needs.
• Develop, execute, and continuously monitor personalized care plans.
• Engage patients through a collaborative, rapport‑building approach.
• Maintain accurate, up‑to‑date professional records of all patient encounters and care plans.
• Provide on‑call coverage, including after‑hours, to ensure patient safety.

Required profile

• Current registration with the College of Nurses of Ontario (CNO) in good standing.
• Valid driver’s licence and reliable vehicle for travel to patient residences.
• Reliability, flexibility, and willingness to travel within the assigned area.
• Experience in community or home‑care nursing is an asset.

Required skills

• Proficiency with patient assessment tools.
• Experience documenting care in electronic health record (EHR) systems.
