Registered Nurse / Practical Nurse – Adult Visiting Program (Central Region)

About the role

VHA Home HealthCare is seeking a part‑time Registered Nurse (RN) or Registered Practical Nurse (RPN) to join its Adult Visiting Program in the Central region of Ontario. As a visiting nurse you will provide high‑quality, client‑centred care in homes, schools, retirement homes and other community settings, supporting clients to live independently.

Key responsibilities

• Assess, monitor and develop individualized care plans for adult clients.
• Evaluate care plan interventions through ongoing reassessment.
• Ensure quality of care by identifying and addressing factors affecting practice autonomy.
• Maintain professional, accountable interactions with clients and families.
• Document all client activities, concerns and care plan updates according to CNO standards.
• Communicate relevant information to nursing supervisors and case coordinators.

Required profile

• Registered and in good standing with the College of Nurses of Ontario (CNO).
• RN or RPN designation with a valid Ontario driver’s licence (G2 or G) and access to a vehicle.
• RPN diploma/degree from a recognized university (for RPN applicants).
• Membership in a professional association (RNAO/WeRPN) preferred.

• Pay per visit: $26.60‑$41.10 (placement on salary range per Collective Agreement) plus a palliative visit stipend.
