Clinical Consultant Pharmacist – Full Time in Ottawa

About the role

CareRx is seeking a Clinical Consultant Pharmacist to join its Ottawa team. You will ensure continuity of care for residents in long‑term care, supportive living and other congregate communities by collaborating with onsite physicians, nursing staff and other health providers.

Key responsibilities

• Act as the liaison between long‑term care homes, retirement communities and CareRx Pharmacy.
• Travel to assigned facilities to build and maintain strong partnerships with leadership and clinical teams.
• Conduct comprehensive medication reviews for residents in serviced communities.
• Complete all required documentation and billing in accordance with MOHLTC, the Ontario College of Pharmacists and other regulatory standards.
• Provide education to residents and staff on health‑related topics, including the MedsCheck program.

Required profile

• Experience working in long‑term care or senior‑care pharmacy settings.
• Familiarity with MOHLTC and Ontario College of Pharmacists regulations.
• Ability to travel regularly to multiple community sites.
