Registered Practical Nurse – Primary Care

About the role

The Registered Practical Nurse (RPN) will join an interdisciplinary primary‑care team at the Weller Clinic, delivering high‑quality clinical care to community members. This full‑time permanent position supports health promotion, disease prevention, and direct patient services within a supportive, equity‑focused environment.

Key responsibilities

• Provide primary health care, including assessments, diagnoses, care planning, interventions, and follow‑up.
• Perform telephone triage, basic laboratory and phlebotomy procedures, immunizations, injections, and wound care.
• Document client encounters in the Electronic Health Record (PS Suite) and contribute to chart reviews and case conferences.
• Order, receive, and maintain medical, laboratory, and harm‑reduction supplies.
• Participate in quality‑improvement initiatives, infection‑control practices, and interdisciplinary team meetings.

Required profile

• Registered Practical Nurse in good standing with the College of Nurses of Ontario.
• Minimum three years of nursing experience in community, public‑health, or hospital settings.
• Current CPR and First Aid certification and a satisfactory Criminal and Vulnerable Persons Check.

Required skills

• Proficiency with Electronic Health Record systems (e.g., PS Suite).
• Phlebotomy and basic laboratory testing skills.
• Computer literacy and use of relevant software applications.
• Telephone triage and client education abilities.
