Registered Practical Nurse – Primary Care (Full-Time)

About the role

The Registered Practical Nurse (RPN) joins an interdisciplinary primary‑care team at the Weller Clinic to deliver high‑quality clinical care to community clients. This full‑time, permanent position supports health promotion, disease prevention and client education within a supportive, equity‑focused environment.

Key responsibilities

• Provide primary health‑care services and telephone triage, following College of Nurses of Ontario guidelines.
• Perform the nursing process—including assessment, diagnosis, planning, intervention and evaluation—for diverse client needs.
• Conduct basic laboratory and phlebotomy procedures such as urine tests, glucometer readings, injections and immunizations.
• Order, receive and maintain medical, laboratory and harm‑reduction supplies, including assembling kits.
• Document client encounters in electronic health records, participate in chart reviews and support quality‑improvement initiatives.

Required profile

• Registered Practical Nurse in good standing with the College of Nurses of Ontario.
• Three or more years of nursing experience in community, hospital or public‑health settings.
• Current CPR and First Aid certification and a satisfactory Criminal and Vulnerable Persons check.

Required skills

• Proficiency with Electronic Health Records (e.g., PS Suite).
• Phlebotomy and basic laboratory testing.
• Computer literacy and use of relevant software applications.
• CPR and First Aid certification.
