Registered Practical Nurse – Community Care (RPN)

About the role

This full‑time, morning shift position is a replacement role for a General Visiting Registered Practical Nurse (RPN). You will provide in‑home care to patients of all ages, helping them stay comfortable and safe in their own residences.

Key responsibilities

• Use appropriate assessment tools and techniques to evaluate each patient’s individualized needs.
• Develop, execute, and continuously monitor personalized care plans.
• Engage patients through a collaborative approach, building rapport and trust.
• Maintain and update professional patient records for every encounter.

Required profile

• Current registration with the CNO as a Registered Practical Nurse in good standing.
• Valid driver’s licence and reliable vehicle for travel to patient homes.
• Reliability, flexibility, and willingness to provide on‑call coverage after hours.
• Passion for community care and a commitment to patient safety.
