Registered Nurse – Referral Management (Temporary Full‑Time)

About the role

The Registered Nurse in the Parkwood Access Office supports referral management and triage for multiple inpatient programs at Parkwood Institute Main. This temporary full‑time role assesses rehabilitation readiness and ensures appropriate patient placement.

Key responsibilities

• Manage referrals and triage patients for inpatient rehabilitation programs.
• Assess rehabilitation readiness and appropriateness of admissions.
• Communicate assessment findings to regional hospitals and community partners.
• Collaborate with interdisciplinary teams and mobilize community health resources.
• Document clinical information accurately and concisely.

Required profile

• Valid RN registration with the College of Nurses of Ontario.
• Current CPR certification.
• Minimum two years recent experience in geriatric assessment and inpatient settings (e.g., ortho, stroke, rehab, GRU).
• Advanced clinical knowledge in rehabilitation assessment and management.
• Strong problem‑solving, clinical reasoning, and independent practice abilities.
• Excellent verbal and written communication skills.
• Knowledge of quality‑based procedures, safety culture, and conflict management.
