About the role

Providence Health Care is seeking a Registered Nurse to join its Hospital at Home program, delivering hospital‑level care to eligible patients in the comfort of their own homes. The role involves providing personalized nursing care, assessing needs, and supporting recovery while working closely with an interdisciplinary team.

Key responsibilities

• Provide personalized nursing care to patients in their homes or hospital settings.
• Assess patient conditions, develop and implement individualized care plans.
• Deliver interventions that promote health, recovery, and safety.
• Maintain a safe, respectful environment for patients and families.
• Collaborate with physicians, therapists, and other team members.
• Share knowledge and uphold the highest standards of nursing practice.

Required profile

• Registered Nurse registration with the BC College of Nurses and Midwives.
• Minimum one year of acute care nursing experience.
• Valid BC driver’s licence for local travel.
• Effective verbal and written communication skills.
• Ability to work as part of an interdisciplinary team.

Required skills

• Cardiopulmonary Resuscitation (CPR) proficiency.
