Registered Nurse – ACT Community Mental Health

About the role

Canadian Mental Health Association Durham provides integrated community mental health and primary care services across Durham Region. The Registered Nurse will deliver nursing and case‑management supports to clients in the Assertive Community Treatment (ACT) program, helping individuals with severe and persistent mental illness achieve recovery goals.

Key responsibilities

• Provide registered nursing services and manage a caseload of ACT clients in community settings.
• Develop, implement and maintain individualized nursing client care plans and conduct comprehensive mental health assessments.
• Facilitate client access to medical, psychiatric and social services, including medication delivery and education.
• Monitor client status, update care plans, and intervene to prevent crises.
• Collaborate with physicians, psychiatrists and other health professionals to arrange appointments and ensure continuity of care.
• Provide on‑call coverage and support to team members as needed.

Required profile

• Registered Nursing degree or diploma with at least two years of mental‑health experience.
• Active registration with the Ontario College of Nurses and RN Association of Ontario.
• Current Medication Training Course completion, First Aid/CPR certification, and a valid Ontario driver’s licence with reliable vehicle.

Required skills

• Proficiency with Microsoft Office suite (Word, Excel, PowerPoint, Access, Outlook).
• Competent use of internet resources for research and communication.
