Primary Care Nurse

About the role

Bayshore Medical Personnel is seeking a full‑time Registered Nurse to provide primary care services in a Montreal clinic. The RN will support members by delivering assessments, education, and chronic disease management to improve health outcomes.

Key responsibilities

• Conduct patient assessments, screenings, and provide healthy lifestyle support.
• Deliver education and chronic disease management for physical, mental, and occupational health needs.
• Develop and implement individualized care plans as part of a multidisciplinary team.
• Provide follow‑up care and manage both walk‑in and scheduled appointments.

Required profile

• Minimum one (1) year of full‑time equivalent experience in family practice, ambulatory care, or a health‑care setting.
• Baccalaureate degree in Nursing from a recognized university.
• Registered Nurse licence in the province of practice.
• Certificate of Basic Life Support (BLS) for Healthcare Provider or CPR/AED Level C.
• Current N95 mask fit certificate (CSA Standards Z94.4, renewed every 2 years).
• Current Workplace Hazardous Material Information System (WHMIS) certification (renewed every 2 years).
