Pharmacist – Drug Use Evaluation & Pharmacy Systems

About the role

Niagara Health is seeking a systematic, analytical, and collaborative pharmacist to join the Pharmacy team in a Drug Use Evaluation (DUE) and Pharmacy Systems capacity. The role focuses on promoting safe, effective, and efficient medication use across the organization while integrating clinical expertise with operational management of the medication supply chain.

Key responsibilities

• Support review, maintenance, and optimization of the hospital medication formulary and related systems.
• Lead and participate in medication‑related contracting activities.
• Provide clinical leadership on drug shortage evaluation, response planning, and mitigation.
• Monitor drug‑use data and key performance indicators to generate insights and drive improvements.
• Interpret medication data for organizational leaders to enhance prescribing, administration, and overall medication use.
• Utilize financial reporting and analysis to support drug budget adherence and data‑driven decisions.
• Collaborate with physicians, clinical leaders, and departments to improve patient outcomes and cost‑effective medication care.
• Chair the Formulary Committee and lead Pharmacy and Therapeutics Committee activities.
• Develop and update drug policies and related informational resources.
• Build and maintain relationships with internal and external partners, including suppliers.

Required profile

• Registered pharmacist with a systematic, analytical, and collaborative approach.
• Experience in drug use evaluation, formulary management, and pharmacy systems.
• Knowledge of medication procurement, inventory control, and contracting processes.
• Ability to travel between sites as required.
