Pharmacist – Community Hospital

About the role

Join Brant Community Healthcare System as a Pharmacist and provide essential medication services across our two‑site community hospital network in Ontario. You will work closely with physicians, nurses, and allied health professionals to ensure safe, effective, and patient‑centred pharmaceutical care.

Key responsibilities

• Accurately dispense prescription medications and verify orders for appropriateness.
• Provide medication counseling and education to patients and caregivers.
• Maintain and manage pharmacy inventory, including controlled substances.
• Collaborate with clinical teams in the acute care and urgent‑care settings to optimise therapy.
• Ensure compliance with provincial pharmacy regulations and hospital policies.
• Participate in medication safety initiatives and quality improvement projects.

Required profile

• Registered Pharmacist licensed to practice in Ontario.
• Pharmacy degree from an accredited program.
• Strong communication and interpersonal skills.
• Ability to work effectively in a multidisciplinary team.
• Commitment to continuous learning and professional development.
