Pharmacist – Antimicrobial Stewardship (Hospital)

About the role

The Clinical Pharmacy Services team at Trillium Health Partners is seeking a dedicated Pharmacist to lead Antimicrobial Stewardship initiatives across its hospitals. Reporting to the Clinical Manager of Pharmacy Services, you will ensure optimal pharmacotherapy for patients while supporting evidence‑based antibiotic use.

Key responsibilities

• Provide patient‑centered care, identify medication‑related problems and collaborate with the health‑care team on solutions.
• Develop, implement and evaluate antibiotic management strategies to improve prescribing, use and cost‑effectiveness.
• Lead project work plans with internal and external stakeholders to advance stewardship goals.
• Participate in clinical rounds, medication reviews and education of staff and trainees.
• Maintain compliance with hospital pharmacy systems and distribution processes.

Required profile

• Baccalaureate degree in Pharmacy or PharmD, licensed to practice in Ontario (Part A).
• Minimum three years recent hospital pharmacy experience, preferably with a residency or equivalent.
• Strong project‑management ability and experience working across multiple organizations.
• Active membership in the Canadian Society of Hospital Pharmacists is an asset.
• Positive attitude, flexibility and commitment to continuous learning.
