About the role
The Canadian Mental Health Association Durham seeks a Nurse Practitioner to deliver primary care and mental health services to complex clients across the lifespan, emphasizing health promotion, education, and disease prevention within the HART Hub residential setting.
Key responsibilities
- Perform comprehensive health assessments and diagnose within RN(EC) scope, including ordering and interpreting diagnostic imaging.
- Provide advanced nursing care, pharmacological and non‑pharmacological treatments, and health education for clients with complex mental health and addiction needs.
- Collaborate with multidisciplinary team to manage urgent health issues, develop care plans, and ensure accurate documentation.
- Contribute to community assessment, program development, quality improvement, and client safety initiatives.
Required profile
- Member of the Council of Ontario University Program in Nursing Primary Health Care Nurse Practitioner Program, in good standing.
- Master’s degree in nursing or related health field and Bachelor of Science in Nursing.
- Current registration with the College of Nurses of Ontario as RN(EC) and membership with NPAO.
- Knowledge of the Mental Health Act, Health Care Consent Act and psychotropic medications.
Required skills
- Proficiency with electronic medical records systems.
- Medication management and accurate medication checks.
