Nurse Practitioner – Long-Term Care (Hamilton)

About the role

The City of Hamilton is seeking a Nurse Practitioner to provide resident‑centered primary health care within Macassa Lodge, a long‑term care home. You will work collaboratively with residents, families, and an interdisciplinary team to deliver high‑quality, evidence‑based care for elderly patients.

Key responsibilities

• Provide direct clinical care, education, consultation and treatment for acute and chronic geriatric conditions.
• Conduct comprehensive functional and mental status assessments in line with the Long‑Term Care Act.
• Collaborate with the Medical Director, attending physicians and the interdisciplinary team to coordinate the circle of care.
• Monitor and manage chronic illnesses, ensuring compliance with Ministry of Long‑Term Care standards.
• Develop and deliver health promotion and lifestyle education to residents, families and staff.
• Participate in ongoing quality‑improvement initiatives within the home.

Required profile

• Baccalaureate in Nursing and current registration with the College of Nurses as a Registered Nurse (Extended Class).
• Experience or certification in gerontological nursing is an asset.
• Strong interpersonal, communication and teamwork abilities.
• Proficiency with computer systems and ability to prioritize in a fast‑paced environment.
• Valid Class “G” driver’s licence (required after hire).
