Mental Health & Justice Rent Supplement Housing Case Manager

About the role

The Mental Health & Justice Rent Supplement Housing Case Manager supports individuals 16 years and older who are involved with the criminal justice system and need stable housing. Reporting to the Housing Coordinator and CEO, the role coordinates rent‑supplement units, builds landlord relationships, and provides case management to help clients secure and maintain housing.

Key responsibilities

• Assess client eligibility and conduct individualized needs assessments for rent‑supplement housing.
• Coordinate with landlords, housing providers, and justice‑related programs to secure appropriate units.
• Develop personalized service plans, monitor risk, and provide side‑by‑side support and counseling as needed.
• Advocate for clients, maintain case files, collect statistical data, and report outcomes.
• Participate in team supervision, staff meetings, and professional development activities.

Required profile

• BSW preferred or equivalent experience.
• Knowledge of mental health, dual diagnosis, and criminal‑justice related housing resources.
• Strong communication, networking, and risk‑assessment abilities.
• Ability to work independently, maintain professionalism, and hold a valid driver’s licence.

Required skills

• Computer literacy.
