Intake & Scheduling Coordinator – Healthcare

About the role

Join Sailor Health as a remote Intake & Scheduling Coordinator, supporting older adult patients who are transitioning to advocacy services. You will act as a trusted guide, ensuring smooth onboarding and ongoing coordination.

Key responsibilities

• Conduct introductory calls with patients interested in advocacy services, following a structured intake process.
• Guide patients through the advocacy onboarding journey with warmth, clarity, and attention to detail.
• Coordinate closely with the advocacy team to ensure seamless transitions and follow‑through.
• Manage patient communications across multiple stages, including follow‑ups, missed calls, and reassignments.
• Maintain accurate records using Google Workspace, EHR platforms (e.g., Healthie), and CRM tools such as HubSpot.

Required profile

• Empathetic communicator who can make patients feel at ease during vulnerable conversations.
• Highly organized and detail‑oriented, able to juggle multiple patients without errors.
• Comfortable learning and using new technology platforms quickly.
• Passionate about improving mental health care for older adults.
