Family Physician – Long Term Care (Part‑time)

About the role
This permanent part‑time position allows you to expand your family practice by joining the long‑term care (LTC) panel at Windermere Care Centre, a 209‑bed facility serving frail elderly residents.
Key responsibilities
- Provide proactive, resident‑centered primary care on a regular schedule.
- Conduct on‑site medication reviews and comprehensive clinical assessments.
- Attend multidisciplinary care conferences and document care plans in the resident chart.
- Participate in the 24/7 on‑call program and after‑hours care as required.
- Collaborate with physicians, nurse practitioners, nurses, allied health professionals, and care‑home leadership.
Required profile
- Family physician licensed to practice in British Columbia.
- Willingness to obtain Vancouver Coastal Health (VCH) privileges and credentials for the care home.
- Interest in long‑term care and commitment to the LTC Initiative best‑practice expectations.
- Ability to work part‑time on‑site in Vancouver.
Required skills
- Proficiency with the PointClickCare electronic medical record system.
