Counsellor (MSW) – Primary Care Team

About the role

Join a multidisciplinary primary‑care team at Community Healthcaring Kitchener‑Waterloo to provide short‑term, solution‑focused counselling for rostered clients. You will work onsite at the Francis Street health centre, delivering up to eight sessions per client per year.

Key responsibilities

• Provide short‑term counselling to individuals and families to maximise independence and function.
• Apply a range of therapeutic modalities such as CBT, DBT and talk therapy, and keep accurate client records.
• Deliver integrated social‑work interventions aligned with the interprofessional team’s goals.
• Document care in the EMR, share information with primary‑care colleagues, and participate in case conferences.
• Collaborate with Client Service Navigators to connect clients with community, health, social and recreational services.
• Offer consultation and education on psychosocial issues to staff and primary‑care providers.

Required profile

• Master of Social Work (MSW) degree.
• Current registration or licensure as a counsellor/social worker.
• Experience delivering brief, solution‑focused counselling in a health‑care setting.
