Clinical Pharmacist

About the role

Mackenzie Health is seeking a Clinical Pharmacist to join its multidisciplinary team. The role focuses on delivering high‑quality pharmaceutical care, optimizing drug therapy, and supporting patient outcomes across hospital and home settings.

Key responsibilities

• Provide drug therapy monitoring, pharmacokinetic consultation, and drug information services.
• Assess, plan, implement, and evaluate medication regimens using the pharmaceutical care model.
• Educate patients on medication use to promote autonomy and reduce non‑compliance.
• Conduct drug utilization reviews to ensure cost‑effective medication use.
• Coordinate acquisition and supply of in‑center and home medications.
• Participate in interdisciplinary care planning.
• Supervise and mentor Pharmacy Technicians, students, and volunteers.
• Maintain compliance with patient and staff safety policies and foster an inclusive environment.

Required profile

• Bachelor’s degree in Pharmacy.
• License (or eligibility) with the Ontario College of Pharmacists – Part A.
• Previous hospital pharmacy experience.
• Proven attendance record and ability to perform essential duties.
• Commitment to Mackenzie Health’s values of excellence, leadership, and empathy.
• Hospital residency or PharmD is an asset.
• Strong communication and computer proficiency.
