Bilingual Nurse Case Manager – Remote (12‑8 pm EST)

About the role

The Bilingual Nurse Case Manager will join Sentrex Health Solutions’ patient support program on a full‑time remote basis. Reporting to the Program Manager, you will work directly with patients, caregivers, and physicians to ensure seamless access to therapy and personalized support.

Key responsibilities

• Adhere to College of Nurses standards and provide nursing care consistent with best practices.
• Assist patients and health‑care professionals with therapeutic area education, program services, and product‑specific information.
• Manage inbound inquiries and conduct outbound outreach to monitor treatment adherence.
• Coordinate patient treatment schedules and facilitate wrap‑around service offerings.
• Support access to therapy by navigating insurance, pharmacy, and logistical barriers.

Required profile

• Registered Nurse, bilingual in English and French.
• Experience in case management, patient navigation, or similar patient‑support roles.
• Ability to work Monday‑Friday, 12:00 pm‑8:00 pm EST.
• Strong communication skills and a compassionate, patient‑focused approach.
