Visiting Nurse (RPN/RN) – Home Health Care

About the role

Bloom Care Solutions is seeking a compassionate Visiting Nurse (RPN or RN) to provide high‑quality, person‑centred care to clients in the Mississauga and Halton region. You will work independently in clients' homes, delivering clinical services, conducting assessments, and coordinating care to support health and independence.

Key responsibilities

• Perform initial and ongoing in‑home assessments of health status, mobility, cognition, and safety.
• Provide clinical care including medication administration, injections, IV therapy, wound, catheter, and ostomy management.
• Conduct diagnostic procedures such as blood glucose and urine testing and manage medical equipment (oxygen, feeding tubes, CPAP).
• Document assessments, treatments, and observations accurately in the electronic health record.
• Educate patients and caregivers on medication use, wound care, symptom monitoring, and equipment management.
• Coordinate with physicians, pharmacies, and other providers to ensure continuity of care.
• Support discharge planning, arrange community resources, and follow up with patients.
• Adhere to infection‑control, safety, and privacy standards (PIPEDA) and participate in quality‑improvement initiatives.

Required profile

• Regulated health professional (RPN) with a minimum of 2 years of experience.
• Current BLS certification and up‑to‑date immunizations.
• Willingness to travel within the Mississauga and Halton region.
• Police Criminal Record Check with Vulnerable Sector Screening and two‑step TB test.
• Excellent communication skills, strong time‑management, confidentiality, and professionalism.
