About the role

We are looking for a compassionate Remote Patient Care Associate to be the first voice seniors and their families hear when they reach out to Sailor Health. You will guide older adults through the intake process, match them with the right therapist, and ensure they feel supported at every step.

Key responsibilities

• Guide patients and caregivers through the intake and onboarding process with warmth and clarity.
• Coordinate matching with appropriate therapists and follow‑up to ensure confidence and understanding.
• Provide emotional reassurance as the first point of contact for individuals seeking mental‑health care.

• Empathetic, clear communicator who can handle difficult conversations with kindness.
• Highly organized and detail‑oriented, able to manage multiple patients simultaneously.
• Tech‑comfortable and quick to learn new software tools.
