Registered Practical Nurse – Primary Care (MD Care)

About the role

Community Healthcaring KW is seeking a Registered Practical Nurse (RPN) to join the MD Care Family Health Organization team. The RPN will deliver patient‑centred primary care across three locations within a 5 km radius, supporting individuals and families in the Waterloo Region.

Key responsibilities

• Complete client histories and prepare exam rooms with necessary equipment.
• Triage client needs by phone or in person and screen minor episodic illnesses.
• Perform basic nursing procedures such as wound dressing, injections, suture/clip removal, and assist with other clinical tasks.
• Provide health education and counseling in line with College of Nurses of Ontario standards.
• Assist providers with prescription ordering, maintain clean equipment, and ensure accurate clinical documentation.
• Stock exam rooms, order medical supplies, and manage inventory controls.
• Maintain contact with pharmaceutical representatives and manage sample inventories.
• Organize client education materials and diagnostic requisitions.

Required profile

• Registered Practical Nurse (RPN) licensed to practice in Ontario.

Required skills

• Basic nursing skills (wound dressing, injections, suture/clip removal).
• Vital signs assessment and immunization administration.
• Health education and counseling.
