Registered Practical Nurse – Primary Care (Full-Time)

About the role

Community Healthcaring KW is seeking a Registered Practical Nurse to join its primary‑care team in Kitchener. You will deliver direct patient care, support interdisciplinary services and contribute to programs that address health inequities in the Waterloo Region.

Key responsibilities

• Take brief client histories and prepare examination rooms.
• Triaging patients by phone or in person and assessing minor episodic illnesses.
• Assist providers with prescription ordering, equipment cleaning and patient flow.
• Perform basic nursing procedures such as wound dressing, injections, suture/clip removal and support other clinical tasks.
• Provide health education and counselling in line with College of Nurses of Ontario standards.
• Carry out non‑clinical duties including inventory counts, supply ordering and distribution.
• Maintain accurate health records and up‑to‑date clinical notes.

Required profile

• Current registration as a Practical Nurse in Ontario.
• Ability to work within the full scope of practice in a primary‑care setting.
• Commitment to serving diverse populations, including refugees and underserved communities.
• Team‑oriented mindset and willingness to collaborate with physicians, nurses, social workers and other health professionals.

Required skills
