Registered Nurse (RN) – Casual/Relief

About the role

Reporting to the HART Hub Lead, the RN will provide primary‑care nursing services for marginalized clients with severe mental illness and addictions across the lifespan. The position is community‑clinic based, serving homeless or precariously housed individuals with a focus on complex care.

Key responsibilities

• Conduct triage, comprehensive mental health and addictions assessments, and develop holistic care plans.
• Administer medications and treatments, including IM injections, perform venipuncture and wound care daily.
• Facilitate access to mental‑health and addiction services, educate clients on medication, and advocate for client‑centred program improvements.
• Document client contacts promptly, support client transitions, and participate in multidisciplinary team rounds.
• Share evidence‑based practices, assist in policy development, provide linkages to other health professionals, and respond to emergencies as part of the Emergency Response Team.
• Adhere to client safety programs, infection‑prevention protocols, and report safety incidents.

Required profile

• Registered Nursing Diploma or Degree with at least two years of mental‑health experience.
• Member of the Ontario College of Nurses and the Registered Nurses’ Association of Ontario.
• Valid First Aid/CPR/BLS certification.
• Knowledge of the Mental Health Act, Health Care Consent Act and psychotropic medications.
• Excellent communication, assessment, organizational and critical‑thinking skills.
• Ability to work independently, manage a variable schedule (including evenings/weekends), and adapt to change.
• Valid Ontario driver’s licence (asset) and proficiency with MS Office 365 and electronic medical records.
