Nurse Practitioner – Long Term Care

About the role

Rhynocare is seeking an experienced Nurse Practitioner to provide comprehensive primary health care to residents in Long Term Care facilities in Oshawa, Ontario. The role focuses on delivering evidence‑based care, collaborating with interdisciplinary teams, and supporting resident safety and quality of life.

Key responsibilities

• Assess, diagnose, treat and follow‑up acute and chronic conditions within full NP scope of practice.
• Conduct comprehensive health assessments covering physical, psychosocial, emotional, cultural and functional dimensions.
• Order, interpret and evaluate diagnostic investigations using evidence‑based clinical reasoning.
• Develop and implement person‑centered care plans in collaboration with residents, families and SDMs.
• Provide clinical leadership for admissions, discharges, palliative care, pain management, dementia care and behavior management.
• Participate in resident care conferences, interdisciplinary rounds and quality improvement initiatives.
• Maintain accurate documentation and support education, mentorship and research activities.

Required profile

• Current registration with the College of Nurses of Ontario (CNO) in the Extended Class (Primary Health Care or Adult).
• Bachelor of Science in Nursing (BScN) required; MSN with NP specialization preferred.
• At least one year of clinical experience in geriatric, rehabilitation or Long‑Term Care nursing preferred.
• Availability for on‑site work, including evenings, weekends and on‑call coverage.
• Knowledge of Long‑Term Care legislation, regulations and standards of practice.
• Physical ability to lift up to 50 lb and perform prolonged sitting, standing, walking and reaching.
