Registered Nurse – Home Care Case Manager (Part‑Time)

About the role

Join a supportive rural health team as a Registered Nurse Case Manager in the County of Flagstaff. You will coordinate home‑care services, build relationships with clients and families, and make a tangible impact on community health.

Key responsibilities

• Assess client needs and develop individualized care plans.
• Coordinate interdisciplinary resources and ensure timely referrals.
• Provide direct wound care, palliative care, and home parenteral therapy as required.
• Deliver client and family education and support.
• Maintain accurate documentation and modify care plans based on ongoing assessments.
• Cover additional assignments and be on call as workload dictates.

Required profile

• Registered Nurse with a valid practice permit (provisional or full).
• Strong assessment, organizational and leadership abilities.
• Experience in wound care, palliative care, and home parenteral therapy.
• Ability to work independently and as part of an interdisciplinary team.
