Registered Nurse – Home Care Case Manager

About the role

Join a supportive rural team as a Registered Nurse Case Manager delivering home‑care services to clients in the Flagstaff County area. You will coordinate care, provide direct nursing interventions, and support families while working from the Sedgewick Community Health Centre.

Key responsibilities

• Assess client needs and develop individualized care plans.
• Coordinate interdisciplinary team resources and external referrals.
• Provide wound care, palliative care, and home parenteral therapy as required.
• Deliver client and family education and ongoing support.
• Maintain and modify care plans based on evolving client conditions.
• Respond to on‑call duties and cover additional assignments as workload dictates.

Required profile

• Current Registered Nurse licence (provisional or full).
• Experience with wound care, palliative care, and home parenteral therapy.
• Strong assessment, organizational and leadership abilities.
• Willingness to work in a rural setting and be on call.
