Registered Nurse – Diabetes Education Program

About the role

The Registered Nurse will deliver community‑based diabetes self‑management education, health assessments and insulin management for adults with type‑2 diabetes in the Kitchener‑Waterloo region. The role works within an interdisciplinary team and collaborates with primary‑care providers, endocrinologists and community partners.

Key responsibilities

• Provide individual and group education, health counselling and behaviour‑change support within RN scope.
• Initiate and adjust insulin therapy according to authorized orders.
• Maintain accurate clinical documentation in line with College of Nurses of Ontario guidelines.
• Participate in planning, implementation and evaluation of diabetes education programs.
• Supervise nursing students and volunteers, offering orientation, coaching and performance monitoring.
• Drive to client sites as needed and work occasional evenings or weekends.

Required profile

• BScN or equivalent with current registration in the College of Nurses of Ontario.
• Minimum three years nursing experience, preferably in community health.
• Experience developing and delivering health education programs.
• Certified Diabetes Educator qualification or equivalent is preferred.
• Ability to obtain a Police Vulnerable Sector Check.

Required skills

• Proficiency with Microsoft Office (Word, Excel, Outlook).
• Experience using electronic medical record systems.
